DOCUMENTS TO INCLUDE IF YOU ARE APPLYING TO REPLACE A PASSPORT WHICH HAS BEEN LOST OR STOLEN:

In addition to the relevant passport application form, photographs and fee:

• A Police Report on the loss
• Completed Form LS01  

Use form LS01 to report any loss or theft of your passport whether applying for a replacement or not.

If your passport is lost or stolen you need to report it immediately to the nearest police station. Once you have informed the police you then need to report it immediately to our office with the full police report and a completed LSO1 form even if you are not applying for a replacement passport.

Remember that your passport is a valuable document. Keep it safe at all times and make sure it does not get lost, stolen or damaged.  

Please note: A passport that has been reported lost or stolen will be cancelled immediately and cannot be used for travel if later recovered.

If a lost or stolen passport has expired then the subsequent application will be treated as a first time application
